About the company: 

We are a 25 years old Health Care company providing chronic disease management and home health services to patients.

Our Telehealth platform is aimed at Chronic Disease Management and designed to accommodate both patients and providers to achieve the best health care outcomes in a very simplistic and inexpensive way.

The system collects vital signs and biological data from chronically ill patients living at home or in a residential facility and transmits it to decision-making clinicians in real-time which allow for instant intervention. This system aims to reduce hospitalization and ER visits and costs involved while keeping patients out of the hospital in a better health condition. The system also involves a few other innovative and unique features to support the homebound and elderly patient population. A school setting for special needs children and a daily COVID screening tool for staff of any company and home health patients and recently a unique home exercise program was added to the platform.

Describe the project.: 

The goal of the project is to convert the established platform programmed in Lotus Notes to one of the following languages: Javascript, MS Sharepoint or Python and utilize a Sequel Server or .net or any feasible secure database.

This software is the smallest of two platforms we have programmed in Lotus notes and have been utilized in our home health organization. By converting this one platform to a more popular programming language and data server, we will have the flexibility to establish a cloud-based software that can reduce risks of local server interruption and have the convenience of the ease of use of multiple technologies such as Bluetooth connections and have a downloadable app to accommodate both iPhone and Android. A web app is already in place for this platform, however, a mobile device app that resides on a phone will provide more convenience for establishing a blog and play platform.

Required expertise/skills:
