Taking the Problem Oriented Medical Record Forward: The Aurora Design, Constellations, Intervention and Planning Approach

Value-Based Healthcare (VBHC) is becoming a leading approach to improving patient and health system outcomes around the world. This research is to develop a new problem oriented record for the VBHC where it can analyze the Patient Progression which is the hospitalized patient journey through the required care events, actions and processes to achieve a health status where the patient can be safely and appropriately transitioned to a lower level of care. Patient progression is directed by the physician orders according to hospital policies and procedures. It will be built on the success of the previous version of the system which called Aurora POMR. The enhanced POMR will have advance smart and recommendation services as well as metrics to measure the patient progression outcome.
