iClinic-HF is one of the specialized modules that is operating in the 12 heart failure (HF) clinics across British Columbia. The major challenge that these clinics face are the limitations and constraints in resources with regards to the staff and funding operating under constraints of a universal healthcare system environment. Further limitations of an effective provincial HF clinic network, and fixed capacity of each HF clinic, necessitates smart solutions to help all patients using specialized care as quickly as possible, minimizing cardiac complications, and maximizing their longevity. Guideline-directed medical therapy (GDMT) is the cornerstone to improving outcomes and reducing the burden of disease and cost. The potential for improving medication optimization can lead to clinical outcome improvements,2 augmentation of clinic capacities, as well as the possibility of developing a tool for commercial uses. Medication optimization requires multiple titrations in HF patients and therefore a collaborative team approach is needed between the healthcare providers. The sequence (i.e., order of GDMT including four classes of medications) of prescription could be different based on patient characteristics as well as the race at which a particular patient manage to be optimized (slow, standard and rapid optimizers).
