Facilitating Service Network Navigation: Developing a Model for Individuals with Autism and/or Intellectual Disabilities and their Families

Adults with Autism (ASD) and/or intellectual disabilities (ID) who seek medical and dental treatments often require specialized interventions that are not always accessible in the community. The Centre for Innovation in Autism and Intellectual Disabilities See Things my Way clinic (STMW) developed a specialized Medical/Dental Clinic to address this important gap. Providing care through a holistic understanding of patients within their broader social environment, the clinic is expanding the breadth of its interdisciplinary care to include a psychosocial component that will help ensure support through a patient/family-centered model of integrated care. This proposed project will focus on the development and implementation of this care model that addresses patients and families service requirements that are often confusing, frustrating and difficult to navigate.
